Great question! According to OSHA Appendix E and OSHA 1910.120(e)(6), certificates issued to individuals who complete HAZWOPER training should include the following:

  1. Student's Name: Identifying the individual who completed the course.
  2. Course Title: Clearly stating the type of training received (e.g., 40-Hour HAZWOPER or 8-Hour Refresher). 
  3. Course Completion Date: Indicating when the training was completed to ensure the certificate is current.
  4. Statement of Completion: Certifying the student successfully completed all course requirements and the CFR (Code of Federal Regulations) the training meets... Note: For HAZWOPER, a statement like "The above named individual has successfully completed the training requirements under 29 CFR 1910.120(e),(q).
  5. Training Provider Information: Including the name and address of the provider offering the course and, per 29 CFR 1926:  The written certification record shall contain... the signature of the person who conducted the training or the signature of the employer.
  6. Certificate Identification Number: A unique ID assigned to the certificate to prevent forgery and ensure traceability.
